Common mistakes when submitting forms:
Liability Release: The camper’s name goes on the first line, and the name of the camp she is attending goes on the second line. Parent/Guardian name is only needed on the bottom of the form.
Medical Information: You may enter “Premier VB Camps” on the second line for “Camp(s)”. Please be sure to check all the boxes, regardless if it is a “Yes” or “No”.
